On your first appointment to Unlimited Performance, your massage therapist will request that you complete a medical history form, including any health issues, previous surgeries, recent injuries and current medications. Your massage therapist will create a treatment plan based on your medical history and specific goals. This process ensures safety and allows us to provide the most beneficial treatment possible.

What should I expect at my first visit?

- Call (815) 748-8900. The sessions are either half-hour or an hour in length.
- Physician referrals may be submitted to insurance companies.
- Self-referrals are asked to pay at time of service.
- Gift certificates are available throughout the year and may be purchased at Unlimited Performance in Sycamore or in the Kishwaukee Community Hospital Gift Shop.
